State in detail all the facts which you believe justify your complaint and the exact code of ethics standard/rule/article(s) the individual has broken. Code of Ethics for each certification can be found at the bottom of this form. The Board has 30 days to follow up with this complaint. If new ethical violations continue from the same provider after this complaint is filed, please email additional information to contactus@nevadacertboard.org.
